The Plan defines acceptable ways you can use to establish that your breast implant was made by Dow Corning. The list is found in Schedule I, Part I of Annex A To The Settlement Facility and Fund Distribution Agreement (Annex A). Additional protocols were also adopted or ordered by the Court.
Unless you submit acceptable Proof of Manufacturer, you cannot receive any payments in the Plan and, without this proof, the Settlement Facility will not process your claim for a Rupture, Explant or Disease benefits. It is very important that all claimants meet this "threshold" eligibility criteria.
