Social Security Number, Proof of Foreign Residency and Attorney Tax Identification Requirement

Claimants must provide a social security number or proof of foreign residency before payment will be issued by the Settlement Facility.  The Settlement Facility will accept any government issued identification from the country in which you reside as proof of foreign residency.

Attorneys in the United States must supply a tax identification number on a W-9 form to the Settlement Facility before payment will be issued.  You may contact Claims Assistance to request a W-9 form.
