If you have submitted your Proof of Manufacturer form and other claim forms along with all supporting documentation and you have received a Notification of Status Letter identifying deficiencies with your claim, you may complete a Request for Review of Additional Information form along with additional records to support your claim and return it to the Settlement Facility for review.
